In 2026, many patients accessing ADHD assessment through the NHS Right to Choose pathway receive their diagnosis within 6–9 months — only to discover their provider has a separate titration waiting list of another 6–12 months. Understanding the process

What is ADHD medication titration?

Titration is not complicated — but it does require careful clinical oversight. Here's exactly what it involves.

ADHD medication doesn't have a universal "correct dose". Each person's brain chemistry is different, and the process of finding the right medication at the right dose is called titration. It involves starting low, monitoring your response, and gradually adjusting — with clinician oversight throughout.

The medications used — typically methylphenidate (e.g. Ritalin, Concerta) or lisdexamfetamine (Vyvanse) — are prescription-only stimulants that require a specialist to initiate and a period of careful monitoring. Done properly, titration takes 6–12 weeks from first dose to stable prescription.

    Blood pressure, heart rate and medical history checked before any medication is started.

    Starting dose

    Weeks 1–2

    A low starting dose is prescribed. You monitor and record how you feel over 1–2 weeks.
